Computer and Internet Acceptable Use Policy/Consent
Acknowledgement that you have read the Acceptable Use Policy and reviewed it with your child.
Request the following current information from your family physician.
- List of required immunizations
- Health Certificate/Appraisal Form Documentation of recent physical exam, completed within last 12 months, or within 30 days of enrollment in the Le Roy district.
- Dental Health Certificate New York State law requires schools to request a dental certificate upon entry to school.
